Report of Test and Inspection
Evidence of Compliance with Dye Testing Ordinance

PROCEDURE:

1. Call Borough Engineer at (412) 921-1110 at least 24 hours prior to performing test. Provide the property address of test. Borough Engineer will prepare a map indicating observation manhole.



2. Pick up form at Borough Administrative Office between the hours of 8:00 a.m. and 4:00 p.m. Monday through Friday. Pay the $35.00 fee when picking up application, payable to Green Tree Borough.



3. If the inspection passes, the completed form must be returned to the Borough Engineer. The Borough Manager will issue the Evidence of Compliance to the Applicant.



4. If the inspection fails, the Applicant may apply for a Temporary Evidence of Compliance. Contact the Borough Engineer to discuss the necessary procedure.



5. Please report any buried or inaccessible manholes to the Department of Public Works at (412) 921-3221.
